The City Applications are in PDF format (Portable Document Format) and require the use of Adobe Acrobat Reader. If you do not already have Adobe Acrobat Reader installed on your computer, you may download and install it free of charge.

You will be able to fill in the forms on-line. Then you may save and/or print the filled-in form.

Note: Adobe Reader 5.0 or higher is required to save filled-in forms.
